Like all health care clinics, there are a number of forms and questionnaires that must be completed prior to your initial consultation with the doctor. We have made these available on our website (click here) and encourage you to take the time to complete them to the best of your ability at your earliest convenience.

If you have previously had an MRI imaging examination performed, please be sure to bring it with you to your first visit. Any medical or health care records of prior treatment that you have copies of would be greatly appreciated!
