We're glad you chose to become a patient and a guest at the Lordex Spine Institute! Like all doctors offices or clinics you'll be required to fill out paperwork that provides us the necessary information to begin your care.

We'd much rather have you complete that 'busywork' now so your time in the clinic will be with the doctors rather than filling out paperwork. It also gives the doctor a chance to review your case prior to your arrival and hence provide a higher level of care to you!

Please take a few moments to fill out each of the forms below to the best of your ability. If you have trouble with any of the questions, just do the best you can and we'll clarify it when you arrive.

Feel free to include anything you feel may be of importance to your doctor-remember, you can't tell us too much! The more we know about you, the better care we can provide.
